The Privacy Policy of OAK Orthopedics, (OAK) describes how OAK or an OAK-affiliated company (collectively, "OAK") handles personal data whether you interact with us on our websites, SMS text messaging, through OAK apps (such as OAK Sports Access or OrthoSync Connect), or in person (including by phone or when visiting our medical offices). OAK’s Privacy Policy does not apply to how third parties define personal data or how they use it. What Is Personal Data at OAK?

At OAK Orthopedics ("OAK", "we", "our", or "us"), we respect the privacy of every visitor, including visits to https://www.oakortho.com, our OAK MyChart portal (available at https://mychart.ibji.com), https://www.hpi-ibji.com, https://orthosync.app and our mobile applications (collectively, "the Sites").

Data that directly identifies you - such as your name - is personal data, and also data that does not directly identify you, but that can reasonably be used to identify you - is personal data. Aggregated data is considered non‑personal data for the purposes of this Privacy Policy.

There may be situations where we cannot grant your request - for example, if you ask us to delete your transaction data and OAK is legally obligated to keep a record of that transaction to comply with the law. This Privacy Policy describes how OAK Orthopedics mobile applications for patients (our "Applications") use, store and transmit information and data. By using our Applications to connect to a healthcare provider, you consent to this Privacy Policy. OAK Orthopedics may modify this Privacy Policy at any time effective upon its posting. Your continued use of our Applications constitutes your acceptance of this Privacy Policy and any updates. Your use of our Applications is subject to the applicable Applications' End User License Agreement.

Text Messaging Communications

When you sign up to receive text messages from OAK Orthopedics, we collect your name and phone number, information about your browser, information about the page you signed up on, and any other additional information you may provide to us. Required Access to Information for OAK Mobile Applications

OAK Orthopedics does not store your location data and will never share any location data with third parties.

  • Location: OAK applications may use your location data to notify front desk staff when you've arrived for an appointment or to suggest healthcare providers that are near you. OAK Orthopedics will not store your location data.
  • Camera: OAK applications may use your camera to take new photos or to capture video for video visits. The photos you take may be used to personalize your account or used as file attachments that are sent. OAK Orthopedics may store data collected from the camera in your medical record.
  • Microphone: OAK applications may use your microphone to capture audio for video visits. OAK applications will not store your audio data.
  • Storage: OAK applications may access your device's storage to read and write files you choose to use in the application. These files may be used as file attachments that are sent to your provider or they may be created from file attachments sent to you from your provider. OAK Orthopedics may store files uploaded from your device's storage in your medical record
  • Bluetooth: OAK applications may access your device's Bluetooth. Bluetooth will be used to detect other nearby devices to help with appointment arrival. OAK Orthopedics will not store your Bluetooth data.
  • Phone: OAK applications may use your phone to call phone numbers displayed in the application. OAK Orthopedics will not store your call history or other call data.
  • Telehealth: OAK Orthopedics may allow you to conduct a telehealth appointment with your healthcare providers, such as a video visit or a phone appointment that may be subject to additional terms and conditions for you to review and accept.

Requirements to Utilize OAK Mobile Applications

To use our Applications, you must have an account with OAK Orthopedics. You understand that while connected or attempting to connect, to our system, we may collect, store, process, maintain, upload, sync, transmit, share, disclose, and use certain data and related information, including but not limited to information or data regarding the characteristics or usage of your device, system, and application software, and peripherals as well as your personal information, location data, and other content. Mobile app requests are handled through the OAK Orthopedics (OAK) secure hosting service (https://apps.ibji.com). This service handles user logins and authentication. While accessing services through OAK's mobile apps, we collect certain information automatically, including, but not limited to, primary account details, the type of device, unique device identifiers, and user-generated content or data.

How We Protect Your Information

The security of your information and data while using our Applications is very important to us. Our Applications employ a variety of technical safeguards to protect the confidentiality, integrity, and availability of your personal information including supporting Transport Layer Security (TLS)/Secure Sockets Layer (SSL) certificate technology and encryption.

In addition, healthcare providers with whom you connect may use a variety of physical, administrative, and technical measures to protect your personal information.

Google Analytics - Cookies

This website uses Google Analytics, a web analytics service provided by Google, Inc. ("Google"). Google Analytics uses "cookies", which are text files placed on your computer, to help the website analyze how users use the site.
